                       2.  Print out the form

                       3.  If paying by check- mail or deliver the form and check

                       4.  If paying by credit card (Visa or MasterCard only)- mail, fax or deliver the form

                       5.  You will receive an e-mail confirmation when your registration is received

                       6.  Receipts are issued at the school.
